The organization’s mission is to recognize opportunities within under serviced communities and serve as a mechanism for social change and development that enhances the quality of life of inner city youth, their family, and community. Our Community Partnership Mentoring Program is a strength based Transformative Mentoring approach to individualized mentoring in which youth 12-21 years old are motivated, provided with referrals, and supported in school, at home, and within their own community. Activities are supported by an individually assigned mentor who provides at least 10 weekly hours. To make this program successful for each participant involved, Committed Community Mentors (CCM) will provide the following resources: * Overall program management and implementation; Development of individualized goal plans for each participant to address goals at school, home, and in the community * Appropriate insurance coverage as required * Trained mentors/instructors with the necessary background checks and clearances; * A lead mentor to participate in weekly “program review” conversations with the community liaison of thirty (30) minutes to discuss successes and areas for improvement; * A family teaming conference to gather interested parties and involve them in the planning process * Individualized Education Plan (IEP) support/ Child Welfare Navigators * Exposure to Local University students; and * Data gathering, analysis and program evaluation services. * Shared Community Space * Consulting CCM unique services will be instrumental in providing direct member and family support to: * Families with limited income * People living in-, impoverished neighborhoods * Homeless children * Children living in single parent households * Youth adjusting poorly in school * Youth coming out of juvenile lockdown facilities or at risk of being incarcerated * High school drop outs * Children who have been abused and/or neglected or institutionalized. * Families with two working parent households
